1. Severe FMR RV > 30 mls or ERO > 0.2 cm2 is associated with 2 fold high mortality
2. Ischemic MR: vent disease, annulus dilatation
3. Cooptation depth is linked to severity of ischemic MR
4. Mitral annulus area is most important
5. Moderate MS is rare after restrictive annuloplasty
6. CRT should be considered in a patient with moderate to severe MR at high risk before surgery
7. Targeted rigid remodeling rings should be considered in surgery in heart failure
8. Mitral clip is to be considered in severe end stage systolic heart failure
9. In many cases ischemic MR 2-3/3 will improve after CABG,
